Step 1: Initial Call and Intake
The first step in our care process is conducting an initial assessment to gather information.Then, we schedule a home meeting with the client and family. During this visit, we further assess the client’s needs and develop a personalized care plan based on their medical history.
Step 2: Personalize Care Plan
Once we’ve crafted a personalized care plan for our patient, we proceed to submit the required documentation to Medicaid.Following this submission, we await Medicaid’s response, which typically occurs within a timeframe of 30 days, upon which approval is granted.
Step 3: Caregiver Match
Once nursing services have been approved by Medicaid, we start matching our caregivers and nurses with the patient. Caregivers and nurses will keep a notebook for all care notes. Also, a supervisory nurse makes regular visits during care hours to ensure quality.
